Care management and support coordination aged care and NDIS
We are experienced in providing case management services. We are timely and responsive and will work towards achieving the best outcomes for you.
As clinicians, we are able to recognise when your needs are changing and when you may need the relevant reviews to support your changing needs. As nurses, we are able to conduct the relevant assessments and write the necessary recommendations to best support your needs.
We can provide all or some of your case management / support coordination needs and are open to discussing what your requirements are. Services include but are not limited to, liaising with suppliers and service providers, coordinating care, liaising with hospitals and medical practitioners, arranging applciations for additional funding and supplements, finding staff for your services and providing you with training and support to more independently manage your funding if this is what you would like to do.

Aged Care:
The National Home Care Average pricing for care management for home care packages is 28%. Many providers in the Cairns and surrounding area charge closer to 33%. At Able To Wellbeing, we know that the more that is taken from your home care package in care and package management fees, the less there is for service delivery for you. That is why we strive to keep our prices low. Please see below for more information.
Just because our fee is low, does not mean your care is compromised. Rather, the opposite! Your package will be oversighted by a Registered Nurse. Depending on which aged care provider you opt to have your package with, will depend on the rate charged. Able To Wellbeing only charge 11% of the package. Our partner provider only charges 13%, making your total rate 24% of the package, well below the National Home Care Average and that of other local providers. You may opt to remain with your current provider, and Able To Wellbeing can sign up as a service provider to assist in your care management locally.
We are timely, responsive and flexible. We do not dictate to you what services you must have, or that you must have services only through our company. That is our point of difference. Many providers, particuarly larger ones, stipulate you must use their staff or contracted providers. At Able To Wellbeing, we listen and if you have providers you are already engaged with or would prefer to use, we will work with them to become verified providers to support your health and wellbeing! On the other hand, if you do not have any preference then we have providers already set up and verified for you to use!
You will have regular check-ins from our staff relating to your care management. Should you opt to move to our partner provider, you will also have a monthly check-in at a time that suits you with our partner provider!
Pricing for home care packages are:
- Are you currently fully managed with a provider? Able To Wellbeing, in partnership with another aged care provider can offer a rate of 24% of your package, meaning more is left for you and services! You can still use your current support staff or service providers, so long as they are agreeable to becoming a verified provider. Whilst transferring your package to a new provider can seem daunting, we make it easy!
- Are you currently self-managing? If you are self-managing and feel that you need more support or would like someone to take on the management for you to give you more time to do the things you enjoy, Able To Wellbeing charge only 11% of the package to take on this work for you. You can remain with your current provider and we will work with them to co-manage your package. Your aged care provider will charge their rate which on average ranges between 15%-18%.
